Please click on the above link to fill out the DOSA Membership Application form. Copies of your supporting documentation, together with your application for membership are required, there is an option to attach them to upload them with your application but if you would like to send them at a later date please send them to to with APPLICATION in the subject line.
Once your membership has been approved, the membership fee is payable immediately*. We will provide you with an Invoice at the time of approval with the applicable amount as wel as banking details. You will be furnished with a unique DOSA Doula Membership/'Practice' Number - please ensure that you use this number as reference at all times. Your details will be available to clients via the DOSA database and you will be able to claim benefits from participating medical aids. You also earn the right to use the credentials "DOSA Accredited Doula" which will indicate to 3rd parties (hospitals, doctors, medical aids etc.) that you are DOSA Accredited. Please allow for up to 10 working days for your application to be processed. If any additional supporting documentation is required you will be informed via email.

*No pro-rata payments are applicable i.e. the whole fee is payable regardless in which month you join. All banking and other payment details will be send to you together with your approval letter. If your application for membership is NOT approved, and administration fee of R50 will apply. You will be supplied with a document clearly indicating what minimum criteria have not been met and you will be afforded the opportunity to re-apply with the outstanding documentation. Membership fees are not refundable.
*ALL TRAINING ORGANISATIONS ARE SUBJECT TO CONSTANT REVIEW TO ENSURE THEY MEET THE CURRENT MINIMUM REQUIREMENTS FOR TRAINING PROVIDERS
